<p>In most massage trainings, the guideline to students is not to massage a woman in the first three months of pregnancy. The reason usually given is that in the first trimester, the risk of miscarriage is too high, but can massage given by a responsible therapist really provoke miscarriage? Should we stop seeing a client when she tells us she&#8217;s conceived even though this is probably when she is most in need of support?</p>
<p>I am curious as to why there&#8217;s so much fear around massage and pregnancy in the first trimester and interested in examining what happens within the woman and foetus during this period, the actual risks around miscarriage, and what sort of massage might be appropriate.</p>
<p><span id="more-3933"></span></p>
<p>Changes in the motherThe first three months of a pregnancy is an incredible time of change; from a single cell, the baby becomes a recognisably human creature measuring 5cm long. Giving a brief outline of the physiological changes during this period will make clear what sort of work is useful. Once the egg is fertilised oestrogen and progesterone levels rise, causing intense pre-menstrual symptoms. In addition there maybe breast tenderness, heightened emotions, nausea and more frequent urination. Relaxin levels are high to soften the ligaments in preparation to carry the extra weight and enable the birth. The ribs begin to flare as lungs and heart become larger and blood volume increases. The progesterone affects digestion and there may be nausea, wind or constipation. Above all, not surprisingly given how much internal change she&#8217;s undergoing, most women experience extreme tiredness. This is an emotionally challenging time too, with worries about miscarriage, the baby&#8217;s well-being, and fears about how it might be to have a new person in the family. The woman&#8217;s identity is also on the change as she takes on motherhood and all that it means to her.</p>
<p>Changes in the babyThe foetus also makes an incredible journey during this time. Once sperm and egg fuse, the single cell starts to divide while travelling down the fallopian tube to embed in the womb. In the second month limb buds and facial features begin to appear and bones begin to form and a heart beat will be visible by scan. By the third month, the organs are fully formed and will mature over the rest of the pregnancy, she can be seen to archher body and move her lips as she starts to practice sucking.</p>
<p>MassageRegarding the massage itself, every woman will come with different needs, so as always we should be guided by her process in the moment rather than her symptom; you may not alter your work so much, but be guided by what she presents from session to session. As there is so much change during this period, and tiredness, nausea and anxiety are common, touch that will help to stabilise and confirm is helpful. The following is a general guideline as to what might be useful:</p>
<p>Matter of fact intentionUsing a &#8216;matter of fact&#8217; touch can be very calming. This would include tangible elements such as speed, direction and depth as well as internal attitude and the quality of relating. The contact would be on the muscle sheath, the rhythm a little like the music that introduces the shipping forecast &#8216;Sailing By&#8217;. The internal attitude would include an awareness of where our boundaries meet and holding a reassuring thought of &#8216;you&#8217;re OK&#8217;. This should help work towards a sense of containment for the client, while avoiding provoking more up-drift.</p>
<p>HoldingIn this technique the therapist uses full hand contact, placing her palms on the client&#8217;s body for a few breath cycles and moving to the next position on the out-breath. This can be focussed on any level that feels appropriate; bone, muscle, skin or aura. The intention is to contain, give boundaries and reassurance.</p>
<p>Membrane MassageThis works on the muscle membrane, the therapist uses the pads of her fingers to move the membrane over the muscle in a soothing waltz rhythm. The intention is to integrate, digest and ground.</p>
<p>Muscle holdingThis is as much technique as intention, using the whole hand to gently grasp the muscle and moving slightly away from the bone, towards the periphery. The intention is to put the client in contact with the strength in her muscles and by association her inner strength: &#8216;I can cope!&#8217;, &#8216;I can do it!&#8217;.</p>
<p>PackingThis is a refinement of holding in the aura and uses a patting motion to confirm and seal energetic boundaries, usually done around four inches from the body or where the temperature/sensation in the hands changes.</p>
<p>GroundingBy which I mean any sort of massage or exercise to encourage the energy into the legs and feet. This could be energetic holding, muscle holding, gentle stretches or rotations.</p>
<p>Working to client&#8217;s boundaryThis can help foster a feeling of safety for the client. It requires very slow, sensitive movement; for example, when doing shoulder rotations, sensing where the resistance is and stopping at that edge, then working gently by trying to avoid triggering resistance. It can also be done verbally by acknowledging the holding to the client.</p>
<p>AbdomenHelping the energy to settle in the abdomen is good, some women like energetic holding here or on the lower back, though it&#8217;s always worthwhile checking to see what feels OK.</p>
<p>Giving spaceA vital part of the work at this time is to give the woman space to experience how she is in this new place; pregnancy brings with it a particular softness and permeability which makes the influence of the outside world and the rush of her internal life especially overwhelming, and it is easy for women to lose touch with themselves. This would include space to talk about herself, space to feel her body and to experience her feelings.</p>
<p>Connecting with babyWork can also start to make a connection with the baby. As movement isn&#8217;t usually felt until 20 weeks or so, this would be in the form of focussing attention, breathing or visualisation. In Chinese and Japanese traditions there is a meridian that connects the heart to the uterus known as Bao mai; this meridian can be tremendously grounding as it provides a link between fire and earth elements. It can be brought into awareness by touch e.g. palming, breathing meditation or visualisation.</p>
<p>Massage styles to avoidThe mother and baby&#8217;s systems are in such a state of flux at this time, work that creates more change and upheaval are to be avoided. This would include vigorous or provocative techniques, particularly on the lower back which might be uncomfortable. As Relaxin levels are high to soften the tendons, strong stretches should be made with a great deal of care, if at all. Lymphatic work is also best avoided at this time as the release of toxins can unsettle the delicate hormonal balance. Stretches, opening work and emptying in the groin area might be best avoided early in pregnancy. The main guideline as with all sorts of massage is for our clients to listen to their bodies and give feedback to the therapist. But as in the rest of our practice we should be guided by our own boundaries to show us what we are willing to work with.</p>
<p> Fear of miscarriageFrom my own experience of pregnancy and from working with pregnant women, I have observed that there is a lot of unexpressed anxiety about pregnancy both in women themselves and in our society. Expecting a baby is usually (though not always) a joyful prospect, but we are not good at allowing our fears a voice. Probably as a consequence of this, we are bombarded with often contradictory advice about what might harm the baby. The first trimester is the period when there is most risk of miscarriage with about one in four pregnancies ending this way. It can be a lonely, painful experience and the grieving process needs time and space. Most women who have suffered a miscarriage never find out exactly what the cause was, and it can be very hard to accept that there was nothing they could have done to change the outcome. Listed below are the five common causes of miscarriage:</p>
<p> 1. A genetic abnormality is the most common cause of about 50% of miscarriages; the baby does not develop normally from the start and cannot survive; however subsequent pregnancies need not be affected.</p>
<p>  2. An imbalance in pregnancy hormones can cause conditions such as polycystic ovaries and irregular periods which can cause miscarriage. There are treatments available for hormonal imbalance.</p>
<p>  3. Structural problems such as a weak cervix, abnormally shaped uterus or large fibroids.</p>
<p>  4. Infection can impact pregnancy, not coughs and colds but a very high temperature, and infections of the blood such as hepatitis, toxoplasmosis andgerman measles.</p>
<p>  5. Blood clotting disorders can affect the flow of blood to the placenta which in turn would deprive the baby of oxygen and nutrients.</p>
<p>It is hard to see how massage can affect any of the above conditions, and it has never been shown as a cause of miscarriage. Like moderate exercise, massage can make a great contribution to the well-being of both mother and baby.</p>
<p>ContraindicationsThere are some instances when massage is not recommended. If a client reports any of the following events she should ask her mid-wife or doctor for advice: Vaginal bleeding (often occurs in a normal pregnancy that is brought to term) Pelvic cramps Abdominal pain Dull ache in lower back Clot-like fluid or tissue from the vagina Fever, weakness</p>
<p>My conclusion is that therapists shy away from massaging women early in pregnancy not because the massage itself may damage, but because miscarriage, as common as it is, is a bereavement and a profound loss that many women continue to feel for the rest of their lives and we are afraid of coming into contact with this loss. Also therapists are afraid of contributing to a miscarriage, or of being seen to have caused one, and just notknowing if this was the case. It is understandable that many professionals choose to work with pregnancy when it enters a more stable phase, but early pregnancy is such a vulnerable period, surely these women would benefit from more support rather than less.</p>

<p>Before you consider including it in your pregnancy health routine, do some research to insure that the massage therapist you choose is trained in prenatal massage techniques, not just massage methods. <p>With programs especially designed for stages in the pregnancy cycle, a prenatal massage can relieve discomfort where you need it and when you need it. Let&#8217;s take a look at some of the benefits of a prenatal massage.</p>
<p>Provides a healthy alternative to reducing stress and promoting general well-being;Relieves discomfort associated with backaches, stiff neck, leg cramps, headaches, joint pain and edema (swelling of the joints);Reduces nervous tension, helping promote a better night&#8217;s sleep;Increases immunity and removal of excess toxins;Stabilizes hormonal levels and balances the glandular system which results in fewer complications during birth and fewer instances of newborn complications, such as low birth weight;Soothes inflamed nerves by reducing muscle tension and sciatic nerve pain;Relieves the effects of depression and anxiety brought on by pregnancy.As mentioned above, the person administering a prenatal massage must be trained to properly hone in on the area affected by pregnancy &#8211; at the right time &#8211; and combat the problem with the correct amount of pressure. Trained therapists use a special support cushion during your prenatal massage to ensure comfort and proper positioning and/or a pad with a hollowed-out area to accommodate your growing abdominal area.</p>
<p>Prenatal massage is not for every woman during pregnancy, but most find it to be a vital part of their healthy routine. If, however, you have any discomfort while your therapist is performing the massage, tell them immediately.

<p>In the past 9 years, I have had 3 normal pregnancies and healthy children. While the pregnancies were all normal and complication-free, I had all the different feelings going through both my mind and body. Between muscle and joint pain, there is also weight gain (in my case, this was quite substantial) causing physical discomfort. I never considered that there was any way of gaining some relief. I now know there is! It is a pregnancy massage. I recently discovered (maybe too late for me) that this is an option when pregnant, in most circumstances.</p>
<p>During pregnancy, we have normal feelings of stress, anxiety, even anxiety, as well as happiness and anticipation. Knowing there is a human being growing within us, we try extra hard to take care of ourselves, as well as the baby. However, some of these feelings can be relieved by a wonderful massage.</p>
<p><span id="more-3875"></span></p>
<p>This can reduce so many negative symptoms of pregnancy, including stress, depression and anxiety. We just have to make sure that we have the appropriate pregnancy specialists, also known as pre-natal specialists. Proper positioning and special pillows are needed to support the growing baby during the massage. The specialist will know not to do any deep tissue massage.</p>
<p>Clothing is optional, some ladies prefer clothing (bra and underwear) others prefer towels or sheets for coverage. One important point is that the personal OB/GYN knows that you are considering pregnancy massage. These massages are good for when you are in labor, and are a great bonding experience for you and your husband. Massages when pregnant are not associated with miscarriages or any other complications. They are good for the mother and even after the baby is born, massages can often soothe a baby with colic.</p>
<p>Many mothers, myself included, are unaware of the fact that massages are possible, even helpful during pregnancy. I now know that massages can help eliminate waste products from the mother through the lymphatic and circulatory system, thus giving the mother more energy. Massages can also help with swollen ankles and legs, which any pregnant woman will tell you, are very uncomfortable. During my three pregnancies, I had the usual symptoms, ranging from sciatic nerve pain, stress on my spine, legs, even feet! The exhaustion I felt by the third pregnancy could have been reduced.</p>
<p>To have a massage while pregnant feels like it would be great. To know that my physical pain, stress and anxiety, as well as hormonal issues could have been alleviated by something so good for me is comforting. A calming, refreshing massage I will be seeking if blessed with another child I will be sure to look for a certified pregnancy massage therapist and consult my doctor. I will not go through another pregnancy without one!</p>

In times of crisis, such as the financial crunch that the country is suffering, it is not surprising that a lot of people become depressed. <p>A BC <strong>RMT</strong> has 3000 hours of training. They have specific training in the physiology and anatomy of pregnancy. There are specific techniques that are used for different stages of the pregnancy cycle. </p>
<p>The bigger question is, “What RMT is a specialist in pregnancy massage?” Technically, RMTs cannot say they specialize in any one treatment or technique. This is a regulatory requirement of our governing body, the <strong>CMTBC. </strong> </p>
<p>To work with this regulation you will see RMTs use words like: “focus”, “area of treatment” or “training in”. To find an RMT that works best for you I suggest looking for a personal referral and research that RMT online.</p>
<p>Here are some questions I suggest asking:</p>
<p>1. Do you do pregnancy massage or prenatal massage? <br />2. Do you have body pillows, electric table, etc…?<br />3. Can I read more about you on the web?<br />4. How many pregnancy or prenatal treatments did you do last week?<br />5. Can I talk to any of the women you treated for pregnancy?<br />6. Are you a Dr. Voder licensed <strong>Manual Lymph drainage</strong> therapist? (These RMTs have post graduate training and should be used if you have swelling in the extremities)</p>
<p>The answers to these questions will give you enough information to make a decision.</p>
<p><strong>What are the benefits of a pregnancy massage?</strong></p>
<p>There are so many benefits to having a pregnancy massage. At no time in your life will you undergo so much change in your body over such a short period of time. Aches, pains and body changes can be greatly decreased when you consult an RMT.</p>
<p>Treatments that respond well to pregnancy massage are: <strong>Sciatica</strong> , headaches, muscle tension, swelling in the extremities and <strong>low back pain</strong> .</p>

<p>There is some controversy about these tables because depending on the tension on the hammock it can lead to not enough support for your belly. This can put undue stress on your abdominal and pelvic ligaments. This can be very serious depending on how far you’re a long and if it has been a difficult pregnancy or not. I highly suggest consulting with your massage therapist to discuss this issue.</p>
<p>Resting on your back can position your uterus to put pressure on the returning veins from the lower extremity. This again needs to be discussed as your pregnancy progresses.</p>
<p><strong>Precautions for pregnancy massage.</strong></p>
<p>Make sure the RMT does a thorough history and assessment before the treatment begins. Clients tend to want to rush to the treatment and they sometime over look very serious contraindications for treatment. <p>So how does one become a licensed massage therapist in the United States? The first requirement is proper education. Courses in massage therapy include an average of 500 hours of study on anatomy, physiology, pathology, kinesiology, massage theory and techniques, first aid and CPR, hydrotherapy, clinical practice, sanitation as well as business, communication, ethical and legal issues. Actual hands-on experience is also required. Schools coordinate with the Commission on Massage Therapy Accreditation (COMTA) to ensure that standards are being met.</p>

<p>Thirty nine states in the US offer registration, certification or licensure for massage therapists, while 38 states specifically require it in order to practice. Thirty two of those states use the certification program of the National Certification Board for Therapeutic Massage and Bodywork which deems the therapist Nationally Certified in Therapeutic Massage and Bodywork (NCTMB) after successfully passing tests. Certification may also be earned through a specified process of proving a certain amount and quality of training and experience.</p>
<p>Registration means a massage therapist has been listed as having met a certain educational requirement. Certification means the massage therapist has met certain higher educational criteria. Licensure is the highest regulatory level and requires a more rigorous process. </p>
<p>The Federation of State Massage Therapy Boards certification exam called MBLEx is accepted by 13 states. Many states will accept State Endorsement when a massage therapist has already been licensed in another state provided that the written and practical examination is equivalent to or more rigorous than their examination, and the licensing authority is as strict or is stricter than their own. </p>
<p>Without a previous license, a massage therapist has to meet the minimum educational requirements and pass the practical and written examination of a state. In certain states the educational requirement may consist of a minimum number of hours specializing in massage therapy if the applicant already holds a license in an approved health related field in the same state. Such fields may include being a medical doctor, registered nurse, occupational therapist, physical therapist, doctor of chiropractic or licensed practical nurse.</p>
<p>Various towns and counties also have their own regulations covering massage therapy and therapists. Most of these regulations guard against the abuse of the profession and may require fingerprinting, clearance from certain diseases including venereal disease, prohibition on making house calls and prohibition on giving massage services to the opposite gender. These are not meant to demean professional massage therapists but, on the contrary, seek to protect the profession from being used maliciously by unscrupulous parties.</p>
<p>Sadly, one of the most common reasons why people go to a licensed massage therapist these days is for treatment after an auto accident.
